TMJ disorders, malocclusion, bad habits and sleep apneaRecent advances in aligner treatment have significantly improved their effectiveness in treating complex malocclusions associated with temporomandibular joint (TMJ) disorders, parafunctional habits, and obstructive sleep apnea (OSA). The integration of modern biomechanics, innovative materials, and customized aligner modifications such as SARA-Splint®, Hemi-Splints, Tongue Shield, and Torque Power Points (TPP) enhances treatment effectiveness. In addition, the combination of aligners with fixed orthodontic appliances and mini-implants (hybrid orthodontics) has improved functional outcomes and long-term stability. This lecture will present an innovative treatment approach that optimizes the functional effectiveness of clear aligners, supported by clinical cases and scientific evidence. Veronika Hanchuk, gnathologist-orthodontist, UkraineCVVeronika Hanchuk, neuromuscular orthodontist, gnathologist Topic: Obstructive sleep apnea syndrome: a hidden epidemic and how to combat itSleep-related breathing disorders (SRBD) are a diagnostic category of diseases that includes such phenomena as: primary snoring, obstructive sleep apnea syndrome, upper airway resistance syndrome, as well as central apnea and sleep-related hypoventilation. Clinically, OSA is characterized by loud snoring, sleep fragmentation, intermittent breathing, frequent awakenings due to suffocation, daytime sleepiness, morning hypertension, decreased blood oxygen levels, and morning headache. In general, about a billion people in the world suffer from OSA. Narrowing of the airways, distal occlusion, secondary adentia of the distal group of teeth, narrowing of the jaws, incorrect position of the tongue, position of the hyoid bone are all dental factors that can not only affect the quality of breathing during sleep, but also be causal factors in the occurrence of OSA. The role of the orthodontist, as a specialist who works with the upper airways, jaw position, and jaw size, is undeniable, both in the early detection of symptoms of sleep disorders, in particular OSA, and in the treatment of OSA manifestations, using intraoral devices for repositioning the lower jaw in adults (in collaboration with a somnologist), as well as treating occlusion pathologies, thereby affecting the width of the airways. Topic: The concept of functionally oriented orthodontics and the use of the GUMMETAL arch: a gnathological, occlusal and postural approach to the treatment of dentofacial anomaliesThe main focus of most orthodontic and gnathological techniques is on the teeth and their relationships. Unfortunately, function is often not one of the main therapeutic priorities. Starting from the analysis of oral functions, we will consider not only local, but also systemic, postural and neurological consequences. From global functional diagnostics, we will move on to the ontogenetic development of malocclusions and methods of their therapy. Gnathological and orthognathic-orthodontic therapy using the multi-loop technique of Professor Sato, adapted and simplified by using the new high-performance orthodontic arch wire GUMMETAL, will be presented. We will examine how this technique allows for the conservative treatment of severe dentofacial anomalies without premolars and without surgical intervention. The rehabilitation concepts presented are applicable not only in adult orthodontic treatment, but also in interceptive therapy and orthopedic rehabilitation. This creates a holistic system for assessing the entire cervical-craniomandibular complex. |
